A lay statement is a written, firsthand account from the veteran or someone who knows them—such as a spouse, friend, coworker, or fellow service member—describing how a service-connected condition affects the veteran’s daily life. These statements help support a claim by offering personal insight the VA might not see in medical records alone.
What to Include:
- Establish how an injury, illness, or mental health condition began during or shortly after military service.
- Your Full Name and Relationship to the Veteran – (e.g., “I served with John Doe in Iraq…” or “I am the spouse of the veteran.”)
- Service or Life Context – Describe what you witnessed during or after the veteran’s service—especially any injuries, behavioral changes, or limitations.
- Specific Examples – Be clear and detailed. Mention how the condition affects the veteran’s ability to work, socialize, sleep, or perform daily activities.
- Honesty and Clarity – Keep it straightforward, respectful, and focused on what you personally observed.
- Signature and Date – Always sign and date your statement before submission.
